Patient Policy

Payments
Aqua Dental accepts payments in the forms of cash, personal/company checks, money orders, MasterCard, Visa, Discover Card, American Express and we also accept Care Credit.

Insurance
If you have insurance, we will be happy to file your insurance claim as a courtesy. However, you will be responsible for your deductible and coinsurance at each visit. Our computer software makes an estimate of what your insurance will cover and your “out of pocket” portion. This is only an estimate and final copay amounts may vary once the insurance has processed your claim. Any remaining balance will be your responsibility to pay.

Interest Free Payment Options
At Aqua Dental we do offer interest-free financing options through Care Credit. Care Credit is not affiliated with Aqua Dental and credit approval is at their discretion. We will assist you in the application process as much as we can.

Financial Arrangements
In order to help with the cost of your dental treatment, financial arrangements may be made with us. Before any treatment is to be done, we can discuss these options with you in a Financial Consult. Any financial arrangements must be made in advance, prior to the start of treatment, and each will be handled on a case-by-case basis at our discretion.

Full Payment Discount
We do offer a discount on cash and check payments made for the full amount of the service, and being paid at the time of service for non-insurance patients. This discount is also offered to those of our patients with insurance when your coinsurance payment exceeds $500.00. These discounts are not applicable with the use of any other offer or coupon.

Finance Charge
Any amount not covered by your insurance company must be paid within 30 days of receiving a statement from Aqua Dental. If the full amount due is not paid within 30 days a finance charge will be added to your account each month until the full amount is collected.

Delinquent Accounts
We will consider an account delinquent when the balance goes unpaid in 90 days without a financial arrangement in place or on accounts with financial arrangements that have defaulted on the agreed upon financial arrangement. Delinquent accounts may be turned over to an outside collections agency for handling. Patients who have had their accounts turned over to collections will no longer be considered active in the dental practice until the balance has been paid. Once this balance has been paid, the patient will then be expected to pay all copayments at the time of their visit.

Return Check Fee
We accept all major credit cards, cash payments, as well as personal checks. If payment is received in the form of a check written by the patient or on behalf of the patient, it is understood that the patient’s account will be charged a return check fee in the amount of $25.00, in addition to any applicable fees your bank may charge.

After Hours Phone Calls/ Emergencies
Aqua Dental is open Mondays 10 to 7, Tuesdays 8 to 6, Wednesdays and Thursdays 10 to 7, as well as some Fridays 8 to 2 for hygiene appointments only. The office is closed on Saturday and Sunday. When the office is closed our answering service will answer any calls made to the office. Over the weekend we are still able to schedule your appointments, or make changes to any existing appointment. Every effort is made to accommodate your scheduling needs.

Missed Appointments
Your time is valuable, and so is ours. In order to accommodate all of our patients' scheduling needs, we kindly as that you give us 48 hours notice if you need to change your appointment. We extend a courtesy to you by sending a reminder via phone call, text or email about your upcoming appointment.

However, if you have reserved time with us, it is your responsibility to contact us if you cannot make your appointment. If you fail to show up, Aqua Dental reserves the right to charge for the missed appointment. A $50 fee will be applied to your account for every half hour missed.

In addition, if you are more than 15 minutes late for your scheduled appointment, we cannot guarantee that we will still be ale to see you, as each appointment is set for an exact amount of time needed for your procedure. This will also count as a missed appointment. Keeping your appointment will help us keep the cost of treatment down.

Prime Time Appointments
In our practice we have found that our late evening (after 5 pm) appointments and our early morning (8 am) appointments are highly sought after by our patients. If you have one of these “Prime Time” appointments and need to reschedule or cancel it is very important that you give us at least 48 hours notice so we can give this appointment to a patient waiting for this time. If we do not receive 48 hours notice, you may be subject to the above missed appointment policy, and if you miss a “prime time” appointment twice without a 48 hour notice to the practice, we will no longer be able to guarantee one of our “Prime Time” appointments for you.

Authorization and Consent for Treatment
Each patient will be asked to sign a consent form at the time of each treatment. We ask that you take a moment to read your consent form, ensuring that your information is correct, and that you understand the treatment to be done. We ask that all parents or guardians of child under the age of 18 accompany their child to their appointment in order to sign the consent form, otherwise treatment cannot be completed and must be rescheduled at a time when a parent or guardian is present.
